Our understanding of and diagnostic criteria for autism, a lifelong condition, have evolved considerably over the last 50 to 60 years. One fall-on effect of this awareness: Many autistic adults overlooked or misdiagnosed in childhood are finally pursuing an evaluation. This population is sometimes called “the lost generation.”

Molly Nikolas, Ph.D., is Associate Professor in the Department of Psychological and Brain Sciences at the University of Iowa and the Principal Investigator of the Iowa ADHD and Development Lab. She is also the Director of Clinical Training of the Clinical Science doctoral program and a licensed psychologist in the State of Iowa. She has authored or co-authored more than 60 peer-reviewed papers on ADHD, with a focus on the interplay between biological and contextual contributors to the development and maintenance of the disorder.  Her research on ADHD has been funded by the National Institute of Health, Shire Pharmaceuticals, and the Society for Research on Adolescence.  She routinely conducts and supervises ADHD assessments for people of all ages through the Seashore Psychology Clinic at the University of Iowa. She has served as the President of the Iowa Psychological Association and as a Member of the Executive Committee for the Academy for Psychological Clinical Science. She has also served as a reviewer on several NIH study sections. Dr. Nikolas is currently an Associate Editor at the journals Assessment and the Journal of Attention Disorders.

ADHD is not a disorder of willpower; it is a condition of dysregulation. Decades of neurobiological research demonstrate that altered signaling in key neurotransmitter systems — particularly dopamine, norepinephrine, and serotonin — contribute to the core features of ADHD. These chemical messengers in the brain shape how we focus, prioritize, feel motivated, regulate emotions, and experience reward.

Dopamine is central to motivation, pleasure, reward processing, and goal-directed behavior. When dopamine signaling is adequate, individuals feel calm, satisfied, and capable of sustained engagement. When dopamine tone is reduced or dysregulated, as often observed in ADHD, the brain compensates by seeking novelty, urgency, or high stimulation. This contributes to distractibility, procrastination on mundane tasks, and the pursuit of immediate rewards over long-term goals.

Norepinephrine supports sustained attention, executive functioning, working memory, and impulse control. In the prefrontal cortex, the brain’s executive control center, dopamine and norepinephrine work together to optimize cognitive performance. When these systems are underactive, individuals may experience task initiation difficulties, emotional reactivity, sensory overwhelm, and mental fatigue.

While not traditionally viewed as a “core” ADHD neurotransmitter, serotonin plays a critical modulatory role in mood, sleep, emotional stability, and behavioral inhibition. Serotonergic imbalance can amplify irritability, anxiety, mood swings, impulsivity, and sleep disturbance, complicating the ADHD clinical picture.

Emotional distress includes feelings of anxiety, low mood, worry, rumination, and self-criticism. Doug Mennin, Ph.D., is a Professor of Clinical Psychology and Director of Clinical Training of the Clinical Psychology Ph.D. Program at Teachers College, Columbia University. He earned his Ph.D. from Temple University in 2001 and has previously held positions in the Department of Psychology at New York University, Yale University, and CUNY Hunter College, where he was Co-Director of the Health Psychology and Clinical Science PhD training program. In his academic role, Professor Mennin has trained numerous graduate students and post-baccalaureate research assistants on diagnosis, assessment, and treatment of anxiety and mood disorders. He has published more than 150 articles, chapters, and books and is the developer of Emotion Regulation Therapy (ERT). He regularly leads workshops to help people better understand and respond to their struggles with anxiety, worry, and depression. Meet the Speaker

Nicole Schatz, Ph.D., is the Clinic Director with the Florida International University Center for Children and Families and a Research Associate Professor in the FIU Psychology Department. Dr. Schatz is also a licensed clinical psychologist in Florida and New York with expertise in behavioral interventions for children and adolescents with ADHD, including behavioral parent training.

Through her clinical work, Dr. Schatz has worked directly with parents of children with ADHD to implement behavioral parent training. Dr. Schatz has also provided clinical training and supervision in behavioral parent training to trainee clinicians. As a clinical researcher, Dr. Schatz’s program of research has emphasized the development and evaluation of behavioral interventions for children with ADHD.

ADHD is not a prerequisite for social anxiety, but many adolescents, teens, and young adults with ADHD do suffer from hidden fears about being embarrassed or humiliated in social situations. These worries about what others think of them appear in the diagnostic criteria for social anxiety disorder as described in the Diagnostic and Statistical Manual of Mental Disorders.

However, this definition of social anxiety excludes other ways in which some people suffer with fears, such as how others may perceive or disapprove of them. These fears are not tied to how they perform in social situations. Instead, these fears relate to aspects of their behavior, thoughts, or wishes that may be seen as inappropriate by others. These fears, sometimes fueled by shame, sound like this: “What would other people think about me if they knew what I was thinking?” This type of social anxiety isn’t talked about much and less than half of those affected seek treatment.

Thomas E. Brown earned his Ph.D. in Clinical Psychology at Yale University and served on the clinical faculty of the Department of Psychiatry at Yale School of Medicine for 20 years while operating a clinic in Connecticut for children and adults with ADHD and related problems. In May 2017, he relocated to California where he opened the Brown Clinic for Attention and Related Disorders in Manhattan Beach.

He has taught continuing medical education courses on ADHD for the American Psychiatric Association for 18 years and has given lectures and workshops in hospitals, medical schools, universities and for professional and advocacy groups throughout the United States and in more than 40 other countries.

Dr. Brown is Clinical Professor of Psychiatry and Neuroscience at the University of California Riverside School of Medicine and is an elected Fellow of the American Psychological Association, The APA Division of Psychopharmacology and Substance Abuse, and the Society of Clinical Child and Adolescent Psychology. Emerging research may explain ADHD’s unexpected associations with inflammatory conditions like allergy, autoimmunity, fatigue, pain, and hypermobility. Scientists have identified genes linked to brain functions that also regulate immune and other bodily functions, indicating a potential connection between these disorders.

Research suggests a surprisingly strong link between ADHD and hypermobility syndrome, a condition in which connective tissue laxity results in a range of physical and mental health problems. Those with hypermobility syndromes often have the following:

  • Dysautonomia: When the autonomic nervous system doesn’t work properly such as in the condition known as postural orthostatic tachycardia syndrome, or POTS
  • Mast cell activation disease: When there is excessive reactivity of the immune system (e.g. allergies) and autoimmune conditions

These interconnected disorders often cause fatigue, sensory issues, pain, anxiety, and mood disturbance, as well as other features commonly seen in ADHD and other neurodevelopmental disorders.

While studies have yet to confirm the role of neurotransmitter-related genes in ADHD, neuroinflammation is emerging as a key contributor to ADHD and other neuropsychiatric disorders. Observational studies link ADHD with conditions such as asthma, rhinitis, urticaria, and food allergy. Substances released by mast cells, a type of immune-based blood cell involved in inflammation, are known to drive neurocognitive impairment, irritability, and fatigue.

  • About the apparent association between inflammation and ADHD, including the likely involvement of the mast cell
  • How disruptions in physical body functions can directly affect brain health and influence or trigger anxiety, depression, fatigue, cognitive difficulties, emotional dysregulation and other features traditionally linked to conditions such as ADHD
  • How findings linking inflammation and ADHD may eventually lead to different ADHD therapies
